AgeWell PACE empowers older adults to remain living at home with dignity by providing accessible, comprehensive healthcare and social support in an inclusive community. Founded in 2021, AgeWell PACE is a non-profit born of the shared commitment of two Federally Qualified Health Centers, LifeLong Medical Care and Santa Rosa Community Health, to address the health and social needs of older adults with complex needs living in Marin and Sonoma Counties. AgeWell opened its first PACE center in Rohnert Park in 2023, the only program of its kind in the area supporting older adults to age comfortably and with dignity in the community.
